About Your First Dental Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and an additional appointment to provide treatment on another day.

Please Assist Us by Providing the Following Information at the Time of Your Consultation:

  • Bring your referral slip and any x-rays (if applicable).
  • Bring a list of your current medical prescriptions.
  • If you have medical or dental insurance, bring your identification cards.
  • When you call our office to schedule your initial appointment you will be asked for your insurance information, including the name of the insurance subscriber, insurance company, employer, and identification numbers (insurance ID or social security number). As a service to our patients, we will attempt to verify your insurance eligibility and benefits prior to your appointment.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If you have had x-rays recently taken, you may request that they be forwarded to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.  Please fax your x-rays to (209) 823-7836.
